Which are correct:

1) This is a device which is to be kept out of the reach of children.

2) This is a device which is to keep out of the reach of children.



3) This is a device which is to be kept at home.

4) This is a device which is to keep at home.

#1 and #3 are correct.
But, better:
This device should be kept out of the reach of children.
This device should be kept at home.
